The Transitions Project provides intensive case management for Arabic and Vietnamese-speaking people who have a history of drug and alcohol related issues and who are leaving prison. Assessment and support are provided pre-release and continue for up to six months post-release.
We offer personal and practical assistance such as:
- Information;
- Referrals;
- Home visits;
- Family support;
- Mentoring.
Anyone can make a referral to the Transitions Project, including workers from Corrections NSW, Community Offender Services, Health Services and community organisations.
DAMEC also provides information and consultation for service organisations on transitional support issues.

A 2015 evaluation of the Transitions Project showed that after 5 months of engagement in Transitions, 86% of participants reported reductions in frequency of illicit drug use, and 28% of participants reduced the number of drugs they used (this included abstinence from illicit drug use). Engagement with support services including housing and AOD treatment was also very high among participants. Read the full evaluation here
